You need to edit your PHP files available in the template which is enabled on your site or the Joomla core files.

Here are the answers to your questions:
1. The text is coming up from Joomla framework
2. Do you mean adding name field to Field attributes?
3. You won't be able to delete field attributes as these are the required parameters to differentiate different pages. Please let us know why you want remove it so that we can understand your requirement and provide some work around to it.

First, there is the text "< class="moduletable ">" above the comment field.
