Module works on detail page only, To display it on listing page, you must have plugin.

Ok I have gotten it to work but I had to disable CAPTURE which is not ideal

If you look at the page link i gave you will see how there are 3 tabs at the bottom of the page
- Ask A question
- Read a Review
- Submit a Review

I'd really like the review form to display on that TAB but because the module doesn't work I can't insert it there?

Yeah I've enabled it and amended the profile but still nothing.
I used to have the module in a tab at the bottom, it would be nice if that still worked. If you email me I'll send you admin login details

There is an option in k2 plugin to show hide review form on listing/detail page, Please check what you have chosen there.
