Are you using some kind of code clean up plugin? If not, Please share your backend & FTP login credentials via Contact us form, We'll look into it else disable that plugin and check if it works then.

Please check the Quiz access settings in backend. You need to check several things e.g. published status, publish start / end date, access level, ip address restriction, number of attempts allowed etc.
