Configuring grouped quantity layout

You get the blank box because the top row is empty in Title and Value.
If you remove this row the blank box is gone.

I am trying to configure this extension and have ended up with an extra blank box, i have obviously set it up wrong so am after some help please!

I havent really achieved more than i could do with just custom fields so hopeful for a simple anser here.
