Order files upload missing radio button options?

Thank you for assisting me before - unfortunately my email was hacked so I need to get the fix for the radio button issue I experienced before. If you could contact me at wendiwebgirl@gmail.com and change my account to that email that would be great. Also I did not see my order for the fileupload plugin in my order history.

I just installed and enabled the plugin however I am not seeing the options for upload and email

I have searched for some options or suggestions to try with no luck can anyone point me in the correct direction - I am sure it is just a setting I am missing somewhere...
