Joomla Category import problem

Hello I have been having a problem importing joomla categories. I have included the csv file so that you can take a look.

Hello,
Please make a few changes in category Profile. Choose "as defined" parameter for level & parent_id and add 1 as default value for both and skip lft & rgt fields.

Please check the attached screenshot. For Title, Alias, path you need to choose the same column ( the only available column ) in your CSV file.

I checked out the screen shot but when I upload my file I do not get the options to make any of those selections. I can only make selections that are on the csv file. Can you give me an exmaple of how the import filr should be setup.
